Harnessing the Power of Chest X-rays and Blood Tests
The COVID-19 pandemic has cast an unprecedented shadow upon healthcare systems worldwide, propelling the urgent need for rapid, accurate diagnostic tools. Artificial intelligence (AI), particularly deep learning (DL) algorithms, has emerged as a beacon of hope in this fight, offering the ability to analyze complex medical data and uncover patterns that may elude human experts. However, the interpretability and reliability of these algorithms remain contentious, especially in high-stakes medical decision-making.
In this comprehensive analysis, we present MultiCOVID, a multimodal AI model that combines chest X-ray (CXR) images and blood test results to achieve early detection of COVID-19 with remarkable accuracy. We delve into the challenges of whole CXR model interpretation, evaluate the performance of single and multimodal models, compare the algorithm’s performance with expert thoracic radiologists, and unveil the interpretability of the model’s decision-making process.
Patient Characteristics and Data Overview: A Comprehensive Landscape
Our study encompasses a diverse cohort of 8578 samples, meticulously collected across four distinct datasets: COVID-19, heart failure (HF), non-COVID pneumonia (NCP), and control samples. This comprehensive dataset provides a robust foundation for evaluating the performance and generalizability of our multimodal AI model.
Upon analyzing patient characteristics and blood test parameters, we observed significant differences in age between the HF cohort and the other three cohorts. This observation highlights the importance of considering patient demographics when interpreting model predictions.
Confronting Challenges in Whole CXR Model Interpretation: Unveiling Hidden Complexities
Previous studies have raised concerns regarding the propensity of DL algorithms to learn non-relevant features to enhance prediction accuracy. To address this challenge, we developed a novel segmentation algorithm that isolates lung parenchyma from CXR images, enabling more focused analysis.
Initial evaluation of whole CXR models revealed a disconcerting tendency to learn spurious characteristics, both within and outside the lung parenchyma. This observation prompted the pivotal decision to segment all CXRs before training models for diagnosis prediction.
Performance Evaluation: Single vs. Multimodal Models – Unveiling Synergies
We constructed various DL models utilizing segmented CXR images and blood sample data, either in isolation or in combination. CXR-only models demonstrated more robust prediction of all four categories (COVID-19, HF, NCP, and control) compared to Blood-only models.
To gain insights into the decision-making process of Blood-only models, we employed Shapley Additive explanations (SHAP), a powerful interpretability technique. SHAP analysis revealed two distinct axes of patient classification: the immune compartment and the red blood cell (RBC) compartment.
The integration of CXR and blood tests using multimodal models yielded a slight improvement in prediction capacity compared to single models. Notably, the joint approach employed for constructing the MultiCOVID algorithm demonstrated enhanced performance across multiple metrics.
MultiCOVID vs. Expert Radiologists: A Comparative Analysis – Unveiling Superiority
To further validate the MultiCOVID algorithm’s prowess, we juxtaposed its performance with the interpretations of expert chest radiologists. The algorithm achieved an overall accuracy of 69.6%, significantly surpassing the consensus interpretation of five radiologists (59.3%).
Remarkably, MultiCOVID exhibited comparable sensitivity to the radiologists’ consensus for COVID-19 prediction, while boasting significantly higher specificity. This finding underscores the algorithm’s potential to minimize false positives, a critical consideration in clinical practice.

Our study unveils MultiCOVID, a multimodal AI model that harnesses the power of CXR images and blood tests to achieve early detection of COVID-19 with remarkable accuracy. The model outperforms single-source models and demonstrates superior performance compared to expert thoracic radiologists.
